I just had this same issue. I tried everything I could and couldn't resolve. Finally, what did help, I pulled the battery, put it back and, while powering up I held the (Back) button down all the way to the boot screen.
At this point, enter "safe" mode. Delete the app that caused the App Permission window, and reboot.
That worked for me. Good luck and I hope that helps.
