HELP:Redirect message

Hi,
When ever i make a call and once the call is connected I get a message on the screen saying " redirected to +91xxxxxxxxxx" this is the same number which i have dialed.
The entire message on the screen is
Connected to Person X or a phone number if the person is not on my contact list
followed by "Redirected to" the same number which I have been connected to.
Anyone has any idea whats wrong?
I tried getting my sim replaced also, and I am from india and my cell is a t-mobile one running on OS 4.5
