Does anyone know why I would have "My Number" displayed three times in separate bars in the phone screen?

I think he means in the "My Number" bar, not the call log. I've seen that issue reported before but don't remember what fixed it. Did you try removing the battery and sim card and reinserting?

That time it worked when the SIM was removed and not just the battery.
