c:\program files\common files\research in motion\apploader
delete vendor.xml
run the loader.exe program
Good luck... You have gotten (for the most part) excellent information here.

Try to reinstall it and "repair" if that is one of the options.

yes! if you use the loader.exe it will recognise the v5.0 file and allow it to be installed, as long as the vendor.xml file is deleted, thankyou very much blackberry forums and NJBlackBerry and Misho and the original John Clark! i shall now be running the os 5.0 on and O2 telefonica device !

If you guys still need help, I have a way to upgrade to OS 5.0 without the data plan. Well at least it worked for me.
1) Download OS 5.0 from your carrier, if your carrier doesn't have 5.0 you can download it from another carrier.
2) If you downloaded from a different carrier then you have to delete the "vendor.xml" file from C:\Program Files\Common Files\Research In Motion\AppLoader folder.
(If you didn't, then skip this step.)
3) After you have deleted the "vendor.xml" file, you have to look for the "loader.exe application" which is in the same exact location of the "vendor"
4) Open the loader application and click next and the application should find the new OS and let wait for it to do its thing.
If you're still stuck/unclear let me know! I'll try to clarify the steps and stuff if you guys are still stuck!
Hope it works!

Most of the time (depending on the OS) Loader.exe will require a data plan, as well. Some of the early versions of 5.0 may load without it.

Installing a 4.6 OS is the only way to get a 4.6 OS folder.

yes its possible, i did it by doing this.
YES! i have successfully loaded .592 without a data plan by doing the following
1. Installed .592 OS to computer
2. Deleted Vendor.xml
3. Opened up DM 5.0.1 and selected Application loader then selected Update Software
it shows OS 5.0 there with a asterisk* basically saying bb data plan users get this upgrade only.
4. I left DM open and went to the Apploader directory and ran apploader.exe it detected my phone and showed my PIN number
5. I opened this program called shrink-a-os and loaded apploader from there also and apploader said PIN UNKNOWN and i selected NEXT (if it goes through you did it right!), it showed 5.0 ready to load with all the check boxes and all that.
6. I then closed DM and the apploader that detected my PIN (NOT the one that said PIN UNKNOWN).
7. Then I disconnected my internet, i read that it prevents vendor.xml from showing up again while its setting up.
8. before starting the upgrade i only selected the language support english check box (one of the first boxes).
9. voila its loaded on to my phone, then afterwards i loaded the apps that came with 5.0.
i read up on the storm forums about this little glitch in the system, basically you connect your phone to DM and youre trying to get apploader to say PIN UNKNOWN that way it is able to detect the 5.0 folder and ready to load the OS.
the first time i loaded 5.0 it failed because i made it load all the apps along with the OS
this time it went through because i just installed the OS itself, then apps later.
i am enjoying threaded SMS and vibrate and ring simultaneously.
P.S. i know my directions dont seem very clean and clear cut but this is exactly what i did to work around apploader not letting me update, no BS. i also did this without wiping with BBSAK.
