It happen to me once, with a 16 gig card. Its usually means the faulty card, there are a lots of fake cards going around. I see that you have already done the tests, but the fault lies in one of the sectors, so unless you have done the long (extended) write test you may not detect it. If you are buying a high density micro sd card (16gb and above) its advisable to go and buy from reliable sources (I am not saying you dint, maybe you just a dud one).
