Lightbulb Calendar and Meeting Invites on Enterprise

An Outlook Exchange server meeting request acceptance message is not being accepted into Outlook from the Blackberry. User received an invitation to a meeting on his Blackberry which he accepted, however when he checked on his Outlook, this meeting seemed to have not updated and didn't show as accepted. He input the diary entry manually, and then ask the meeting organiser to resend the invitation so it shows all the attendees.

We have tried to run Wireless Calendar Reconciliation but the meeting is still not updating on Blackberry or on Outlook.

Is it possible this meeting request has become orphaned? Can anyone explain how this happens?
