Enterprise Activation Completes; however, it starts up again for no apparent reason.

Hello all,
My organization is experiencing some strange behavior with enterprise activation and users that have replaced their blackberry for whatever reason.
Case 1: User A replaces his old bb curve with another bb curve (or any bb). The user's old blackberry is deprovisioned on the BES and the new/replacement blackberry is provisioned. Enterprise activation runs and completes on the new/replacement blackberry. Everything about the new blackberry appears to functioning fine. Then a few hours later (or even a day or two), Enterprise Activation starts up again for no apparent reason. This problem has caused us unbelievable amounts of headaches trying to figure out what's causing this to happen. It happens on all blackberry models. And it only happens with "replacement" devices. Our organization has consulted with the providers, BES admins and possibly RIM (not sure).
1. We've had the providers remove the users account from their network and re-add them.
2. We've had the BES admin delete and re-create the user's account on the BES server.
3. We've wiped the devices.
Can anyone give me a clue as to what would cause enterprise activation to start up again on a fully activated and functioning blackberry?
