RSA Secure ID not showing up

I have a user with a Verizon 8330 curve. He's running OS 4.5.0.77 and has RSA Secure ID v2.1.1 installed on his blackberry. He did something last night where the icon to the RSA is now gone. It's not hidden or moved to another folder I already checked those. The app still show's installed on his blackberry. I tried uninstalling and reinstalling the software, resending the service books, re-registering it with the network and I still can't get the icon to show back up.
Any suggestions?
I can't upgrade him to v3.0 of RSA because in our test with the 8330 v4.5 it causes the blackberry to reboot whenever I launch the program.
