Looks like Hot Fix 1 is out now. From the Release Notes it shows two fixed issues.

Server version 4.0 SP4, MAPI stopped responding.

Previously, if a user who resided on a Microsoft Exchange version 5.5 messaging and collaboration server modified an
appointment on the BlackBerry device, a duplicate appointment appeared in Microsoft Outlook®.

Service pack 4 was for exchange 2003 service pack 2 support. I was going to install it but I was glad I didn't yet.

We are running a dual 5.5 and 2003 environment. I stoppped on the upgrade of 4.0 to service pack 1 only. I knew there was issues with sp1 for bes 4.0 but atleast the end users are working.
